All new patients must bring:
  • Current insurance card and photo identification
  • Current glasses and contact lens boxes if applicable
  • All minors must be accompanied by a parent for the first visit
 All new patients must complete:
  • Medical History Form
  • Privacy Policy Form
  • Financial Agreement Form

You may complete the form(s) by:

  • Downloading, completing and bring the forms on your first visit.
                                      - OR -

  • Fill the forms out in the office, please arrive 15 minutes prior to appointment time.
